Looked at as data, a Medical Certificate is a small structured record (parties, dates, officials, registry IDs) wearing a paragraph as a disguise; the translation must restore the record exactly, not paraphrase the disguise. Almost every rejection of a Medical Certificate we are asked to fix afterwards comes back to one of these five patterns:
